ONLINE STUDENT INFORMATION SHEET

This University of Northern Colorado online course is offered as part of the statewide coordinated program of Extended Studies instruction under the guidelines established by the Colorado Commission on Higher Education.  It also adheres to University and Office of Extended Studies guidelines as follows:

REGISTRATION:  You have registered for a credit bearing course and the grade you earn will be recorded on a University of Northern Colorado official transcript. You may take this course for no credit by notifying our office at 1-800-232-1749 within 24 hours of the start of the class. When auditing a course, you must still pay full tuition and fees.

TUITION:  You will be billed by the UniversityÕs BursarÕs office for the amount of your tuition payment and any additional fees.  Accounts not paid in full by the end of the month will be assessed a monthly 1.75% finance charge.

WITHDRAWALS AND REFUNDS:  To withdraw from this course, you must telephone the Office of Extended Studies, 1-800-232-1749.  Non-notification of withdrawal will result in full tuition being assessed.

Withdrawal fees are assessed as follows:

$30 if you withdraw between the class decision date and the first class

$70 if you withdraw after the first class and before 50% of the class is completed:

No refund will be given after completion of 50% of the course.

GRADES AND TRANSCRIPTS:  Grades are recorded on your UNC transcript in the term that your course ends.  Transcripts should be available from the Records Office two weeks after the course has ended, if the registration and grade have been received by the Office of Extended Studies for processing by a deadline specified to the instructor.  You must request your official transcript through the UNC WEBSTER (WEb Student Educational Records).  Log onto:  www.was.unco.edu.  You will have to enter your Bear # and your 4-digit SIN (Student Information Number).  This SIN is automatically generated as your birthday month and day. You will be required to change your SIN the first time you use Webster. If you have created a SIN and have forgotten it, you will have to call the RegistrarÕs Office at 970-351-2231 to change it.  The RegistrarÕs Office can also take telephone orders of transcripts, IF you come in person to pick it up.  You may contact our office to verify that your grade has been posted before requesting transcripts or request that the transcript be held until a grade for specific course you are taking has been recorded.  Students with special deadlines should contact the Office of Extended Studies concerning the exact date a transcript will be required.

SCHEDULE CHANGES:  The Office of Extended Studies reserves the right to cancel courses, reschedule courses, or change instructors.  Every effort will be made to inform the student of changes.

CHANGE OF ADDRESS/NAME CHANGE:  It is the studentÕs responsibility to contact the RegistrarÕs Office in the event of an address or name change at 970-351-2231.

INSTRUCTIONS FOR LOGGING INTO AND USING BLACKBOARD

Read the following for username and password information:

Within 24 hours of us receiving your registration form a Bear Number will be assigned to you.  At that time you will be able to officially login to the course.  (Four digits of the Bear Number along with parts of your last name will make up your UNC email address).  Information is included if you want to forward your UNC email address to your personal email address.

Once you get logged in, you will see your Blackboard course page with MED 678 listed under ÒCourses in which I am participatingÓ. You will need to click on the link to get into the course.

To navigate through the course, use the buttons on the left of your page. Try to use these buttons or the Òbread crumbsÓ (the links listed at the top of the page) to move around in your course, rather than using your ÒBackÓ browser button. At times using your back browser button will give you an error message; Òthis page has expired.Ó

You can go into ÒPersonal InformationÓ to change your email address and add any information that you wish to be visible to others on the Blackboard system. If you donÕt use your @blue account and you have not forwarded it to an email account that you access regularly, you will want to do this immediately. When the professor or other students email from within the Blackboard course, you will receive that email on the system your personal information is set for.
